FutureBright Care Navigator Inc. (“Care Navigator”) provides medical care co-ordination services which assist individuals in navigating the health care system and obtaining medical help. FutureBright Insurance Group (“FutureBright”) is a company related to Care Navigator. FutureBright distributes critical illness, accidental death and dismemberment, and long term care insurance.
CareNavigator and FutureBright (“we” “us” “our”) want to inform individuals about why we need their personal information and how and why we handle their personal information.
We obtain consent to the collection, use and disclosure of personal information, except where inappropriate.
We state the purpose for which information is collected, used or disclosed at or before the time information is collected.
When we do collect personal information, we collect only that personal information which is necessary.

An individual does not have to provide such optional information in order to obtain our products and services or any upgrades thereto.

We do not sell our customer lists. However, information is shared between Care Navigator and FutureBright for marketing purposes. This sharing of personal information is optional.

We try to ensure that the information we collect and use is accurate, up-to-date, and as complete as possible. However, we rely on individuals to disclose all material information to us and to inform us of any changes.

If you apply for a Care Navigator service or a related FutureBright insurance product, your personal information will be forwarded on to the service provider or insurer, as the case may be. We will not keep copies of this information other than the information you provide us to process service fees or premiums.

We may charge a reasonable fee in advance for copying and sending information requested and to which there is a right of access.

FutureSafe is group insurance with features that are different from other available individual products. In particular, you should note that the insurer can cancel the group policy in which case your certificate of insurance will terminate, upon notice being provided to you. At least 30 days advance written notice of termination will be provided to you at the address we have on file for you.
The insurer can change premiums upon advance notice being provided to you, although no one insured person will be singled out for a rate change. Rather, premiums may be changed for the group based on the experience of all certificate holders. This flexibility is necessary as FutureSafe is a guaranteed issue product for policy amounts under $25,000 which is available at very competitive rates. “Guaranteed Issue” means that the insurer does not do any medical examination or medical review in determining if a person is eligible and any person who meets general eligibility criteria, such as age, will be accepted. Consequently, the ability to change rates is required should the initial low rates available not be sufficient to support the claims experience resulting from the group as a whole.
It is important that you understand these highlights and all the terms and conditions of your coverage which are set out in your certificate of insurance. There are other limitations and exclusions which apply. You should read the certificate very carefully so you understand both the advantages and disadvantages of FutureSafe.
The Importance of the Information You Provide The insurer relies very heavily on the information and statements you provide in your application to enroll. Any material misrepresentation, including, but not limited to your age or that of your spouse shall render void any insurance certificate issued. You must complete your own application for insurance or that of your spouse. A failure to do so renders your insurance policy void.
The Effective Date of Coverage Coverage comes into effect on the date we receive your application to enroll, but ONLY if you pay your first premium when due. You must continue to pay the applicable premium to continue to keep your insurance in effect. We will send you confirmation in the mail, if you don`t receive it, we did not receive your enrolment and you are not covered.
Replacing other Insurance FutureBright has some great features, like great rates and no medical examination for enrolment, however, FutureBright should not be purchased to replace existing insurance coverage you may already have in effect.
